What's Needed: - Print and bring this completed [INFANT MASSAGE CONSENT FORM] with you.
- Dress in comfortable layered clothing, you will be sitting on a floor-mat in a warm room.
- Arrive 10-minutes early with your baby, what you would normally pack for a typical outing, a baby blanket, and an infant size doll or stuffed animal.
- The office is accessible by elevator.
